128 A single-arm pilot study of an adapted Serious Illness Care Program for older patients with acute myeloid leukemia and myelodysplastic syndromes

OBJECTIVES/GOALS: We adapted the Serious Illness Care Program (SICP), an evidence-based intervention designed to promote early serious illness conversation, be delivered via telehealth for older patients with acute myeloid leukemia and myelodysplastic syndromes. The purpose of this study is assess feasibility usability intervention. METHODS/STUDY POPULATION: are conducting a single-arm pilot SICP that AML or MDS (>=60 years) their caregivers (if available). includes: 1) Patient preparation pamphlet: sent patient prior visit clinician, 2) Geriatric assessment: completed by team provided clinician patient, 3) A 30-60 minute primary oncologist oncology advance practitioner, 4) Conversation Guide (SICG): used during elicit values, 5) Family guide: following help patient’s share values family, 6) Electronic medical record note template clinicians document patient. RESULTS/ANTICIPATED RESULTS: hypothesize will feasible usable. based on retention rate (percent who consent complete visit); >80% considered feasible. Usability assessed using questionnaire; average score >5 Other measures include psychological health, care planning engagement, quality life, disease understanding. plan enroll 20 in study. To date, 11 have consented participate, 10 scheduled visits, 9 7 post-intervention qualitative interviews, 4 surveys. DISCUSSION/SIGNIFICANCE: telehealth-based may patient-reported outcomes, end-of-life experience MDS. Results from inform development clinical trials testing impact patient- caregiver-reported outcomes.
